I, the undersigned, do hereby release Cedar Hill Baptist Church, its leaders and membership from all claims and actions, present and future, arising out of injury or damage to said child. Should emergency medical treatment be necessary, I authorize Michelle Gregory-VBS Director, or Dr. Eric Taylor, Pastor to act on behalf and approve appropriate treatment. By signing this form, I authorize any photo, likeness, image and/or name of said child to be used for church publications, including websites. I leave the choice of these images to the discretion of the church staff, knowing that upholding the honor, integrity and character of my child will be strictly enforced.
